The purpose of this form is to allow all teachers and staff to feel empowered in their ability to reach out and get help with behavioral consults and to gain insight into the behavior that is occurring within your classroom or setting.
While filling out this form we ask you to think behaviorally. Through this process, we are able to gain valuable data to allow us to set up and schedule meaningful observations. This form is the first step in the referral process for behavioral consultation. This form is confidential and goes directly to the district behaviorists, Pam Fasciani and Kate Hartley. Form responses will be checked every three days and one of the behaviorists will in contact with you about your request.
